Safety and Health Officer Report

Most of the monthly report was not accepted by the DOSH because did not meet the criteria which contained in Regulation 19, the Occupational Safety and Health (SHO) Regulations, 1997.

Among the main reasons of report is not accepted by the DOSH are:
  1. Too many attachments included in the report, such as minutes of meetings, records of workplace inspections, records of occupational safety and health training, records of occupational safety and health program;
  2. The report provided too thick and detailed;
  3. No comments or suggestions for the proposed improvements or changes in the interest of safety and health at work.
Every Safety and Health Officer (SHO) who is registered under DOSH is required to provide occupational safety and health reports for workplace before the tenth day of each month. This requirement are consistent with the Occupational Safety and Health (Safety and Health Officer) Regulations, 1997. The report shall be made in form of SUMMARY.

Below is the contents of which shall be provided in the report:-

  1. Any action to be taken by the employer in order to comply with the requirements of the Act or any regulations made under the Act;
  2. method of establishing and maintaining a safe and healthy working condition in the place of work;
  3. the number and types of accident, near miss accident, dangerous occurrence, occupational poisoning or occupational disease which have occurred in the place of work;
  4. the number and types of accident, near miss accident, dangerous occurrence, occupational poisoning or occupational disease which have occurred in the place of work including the number of persons injured either incurring lost time injury or no lost time injury;
  5. any machinery, plant, equipment, appliance, substance or process or any description of manual labour used in the place of work which is of such nature liable to cause bodily injury to any person working in the place of work;
  6. any machinery, plant, equipment, appliance, or any personal protective equipment required for the purpose of minimizing any such risk;
  7. recommend any alteration to be made to the structure or layout of the place of work in the interest of the safety and health of the persons employed therein;
  8. any work related to safety and health which has been carried out by any person, or group of persons, engaged by the employer in order to promote safety and health in the work place;
  9. any outstanding matter arising from the previous report;
  10. any other matters related to safety and health of persons working in workplace.

Examples of simple SHO report and can be accepted by DOSH.
